StephenWeinstein: 1. There are several forms called a "tax return form". The most common is Form 1040.
2. There no such thing as a "1044 form". I think you meant "1040" (see above).
3. W-7 = Form that you send the government in order to apply for an Individual Taxpayer Identification Number. This form is rarely used by U.S. citizens because U.S. citizens normally use their social security number as a taxpayer identification number. If you do need this form, download it from http://www.irs.gov/pub/irs-pdf/fw7.pdf, fill it out, and send it to the IRS. However, I think you meant either W-2, W-4, or W-9. W-4 and W-9 are forms that you fill out and give to company that is going to pay you (employer, bank, etc.) before they pay you. W-2 and 1099 are forms that these companies give you later showing how much your were paid so that you can report it.
4. The form that you filled out would have be a W-4 if you are an employee or a W-9 if you are an independent contractor.
5. If you are an employee, your employee must give you a W-2. You do not have to ask for it. If you are an independent contractor, the company may or may not be required to give you a 1099; however, you do not need it in order to report your income.

v b: The word employer and form W-7 for the ITIN cannot be used in the same context. If you have an SSN (which you must have to work), you don't need an ITIN.
